Order by Email using a Purchase Order
Schools administering ELPA assessments using the CAI platform may purchase headsets via Purchase Order.
Instructions and Notes
- To place and order, download the order form using the link above.
- Please complete ALL entries on the form.
- A Purchase Order number must be provided.
- Orders will be shipped via UPS Ground so cannot be shipped to a PO Box.
- Incomplete information will delay processing of your order.
- Review the Headset Return Policy before placing an order.
- Email completed form to elpa21headsets@cambiumassessment.com.
